This is the case Study for the screening tool: The patient is a 12-year-old white female who came to the ER about self -inflicted overdose. She said the reason why she took the overdose was to harm herself. Her mother was present at the time of the assessment. The patient refused to make eye contact with her mother and kept her head down almost the whole time during the discussion. The social worker was present and took notes about the cut in both patient’s wrists and inner thighs. The patient did not want to give more information as her mother was in the room. The social worker noticed why the patient did not want to provide more information; she politely asked the mother to leave the room. After her mother left the room, the patient became more open with the conversation. She explained the reason that led her to overdose herself. She stated that she was 9-year-old where her stepfather was sexually assault her while her mother was at work. She said that her mother is not aware of the situation at the moment because her stepfather threatened to kill her if she informs her. She has a sister who is turning 9 this month, and she reported that her stepfather is abusing her sister. She said that she becomes extremely depressed, and she recently found out she is pregnant. She is worried about her mother found out her pregnancy that is the reason she decided to end her life.
The patient stated that she cut herself to relieve stress. She said that she started to do it to herself after the abuse occured. She feels hopeless, embarrassed, and ashamed to her mother. She feels like if her mother is aware of what happened, she will be on her stepfather’s sides not on hers. She said that she feels like if she successfully commits suicide, it will be better as she will not be going to embarrass her mother. She said that her mother is a full-time nurse, and her stepfather is unemployed. She said since the past few months, her mother has difficulty to pay her bills, she does not want to put more stress on her.
